A healthy 35-year-old man presented with a short history of left arm weakness and facial droop as well as a right-sided headache and neck stiffness. He later collapsed with left hemiplegia. MRI confirmed multiple embolic infarcts in the right middle cerebral artery territory. Additional imaging revealed an elongated right styloid in close proximity to a 15-mm internal carotid artery dissection, leading to a diagnosis of stylocarotid Eagle’s syndrome.
