A 66-year-old man with diabetes, hypertension, and tobacco use had right-sided hemiplegia 1 year before he developed Foix-Chavany-Marie syndrome acutely 2 days before admission. CT of the brain showed an acute left perisylvian infarct and a chronic right internal capsular anterior limb infarct (shown in blue arrow and red circles).
