This CT of the lumbar spine was requested for a 58-year-old female who presented with a history of chronic back pain. (A) Sagittal bone and (B) soft tissue windows demonstrate a focal disc herniation at the L5-S1 level (black arrow). This disc herniation is paracentral to the left (black arrow) best seen on (C) axial bone and (D) soft tissue windows. There is no significant spinal stenosis. Facet degenerative changes are also noted, especially on the right. (Contributed by Dr. Diana Gomez-Hassan.)
