T2-weighted sagittal MRI of the lumbar spine shows the right L4 spinal nerve within the corresponding neuroforamen (straight arrow). The neuroforamen is partially obstructed by a hypointense mass inferiorly (dashed arrow), corresponding to disc material. The amount of hyperintense fat within the neuroforamen is reduced, and the configuration of the L4 spinal nerve suggests deformation. (Contributed by Kristine Ann Blackham MD.)
