This 39-year-old man had ophthalmoplegic migraine episodes at 13 years of age. He had a normal cerebral angiogram. Ophthalmoplegic migraine recurred at 35 years, and multiple attacks left him with a permanent pupil-involving third nerve palsy. Repeat angiography was normal. (Contributed by Dr. James Corbett.)
